We'll build the Perl Script to go with any form you need a script built. You make an example for what the output page(s) should look like, and we'll make the input show up where it should.
Full Form:
If you need the form to be built along with the Perl to go with it. We would still like to see an example of the output page(s). This is not necessary, although, you will incur an additional design fee of $5.00/hour for the design of the page(s).
